Physical Therapy CRM for Outpatient PT Clinics
A physical therapy CRM helps an outpatient clinic win on two fronts: turning physician referrals and direct-access inquiries into evaluations, then keeping patients coming until the plan of care is finished. GoHighLevel USA delivers it as a done-for-you GoHighLevel setup that sits beside your EMR, not in place of it.

Physician referral and direct access
Plan-of-care visit reminders
BAA-aware setup
Texting registration handled
Physical Therapy CRM Features We Set Up
Each piece targets a specific leak in the outpatient PT journey, from the referral fax to the final visit.
Referral Intake
Physician referrals get logged against the referring provider, so your team can see which offices send patients and which referrals never booked.
Direct-Access Booking
Self-referred patients book an evaluation online from your site or ads, with intake questions limited to what scheduling actually needs.
Arrival-Rate Reminders
Visit reminders and confirmations go out by text and email, and cancellations get a prompt offer to rebook inside the same week.
Missed-Call Text Back
When the front desk is busy with check-ins, callers get an automatic text so the conversation continues instead of going to voicemail.
Discharge to Wellness
Discharged patients can be invited into cash-pay programs such as wellness visits, small-group classes or performance training you already offer.
Referral-Source Reports
Reports show leads and evaluations by referring physician, so outreach to doctors is based on real numbers rather than memory.

Physical Therapy CRM Playbook for Outpatient Clinics
Where the CRM stops and the EMR starts, and how to set it up for referrals, direct access and plan-of-care completion.
Two Front Doors: Physician Referrals and Direct Access
Outpatient PT clinics get patients through two very different doors. A physician referral arrives with an order and a referring provider who expects updates. A direct-access patient found you on Google, a gym partner or a friend and has no referral at all.
Treating both in one list hides what matters. We set up a separate pipeline for each, because the follow-up, the paperwork and the KPI are different.
- Referral pipeline: referral received, patient contacted, evaluation booked, evaluation completed, plan of care active, discharged
- Direct-access pipeline: inquiry, screening call or online booking, evaluation booked, evaluation completed, plan of care active, discharged
The referral pipeline is judged on how many referrals turn into a first visit. The direct-access pipeline is judged on cost per evaluation and speed to first contact.
Keeping referring physicians in the loop
Referring offices want to know their patients were contacted and seen. In a physical therapy CRM, each referring provider is a contact record linked to the patients they sent, so your liaison can see which offices are active and which have gone quiet.
Clinical progress reports still go from the EMR through your normal channels. The CRM is for relationship work: thank-you notes, lunch-and-learn invitations and a simple count of referrals by month.
Direct Access Rules Change Your Booking Copy
APTA reports that all 50 states, the District of Columbia and the U.S. Virgin Islands provide some form of direct access to physical therapist services. The limits differ by state, with some allowing unrestricted access and others capping the visits or days before a referral is needed.
That affects your funnel. A clinic in an unrestricted state can invite patients to book straight in, while a clinic with provisional access should explain the time or visit limit in plain words. APTA’s direct access by state page is the place to check your state, along with your state practice act.
Payer rules can still require a referral even where state law does not, so the booking page should tell patients to check their insurance. Confirm the details for your state and payers with counsel; this guide is not legal advice.
Physical Therapy CRM vs EMR: Who Does What
PT EMRs such as WebPT, Raintree and Jane run documentation, scheduling and billing. They remain the clinical record. A physical therapy CRM handles everything that is about winning and keeping the patient relationship.
| Job | EMR (WebPT, Raintree, Jane) | GoHighLevel CRM |
|---|---|---|
| Evaluation notes and plan of care | Yes, system of record | No |
| Billing, claims and authorizations | Yes | No |
| Ad, website and phone lead capture | Limited | Yes |
| Referral-source tracking by physician | Varies | Yes, as pipeline and report data |
| Two-way texting, email and web chat inbox | Varies | Yes |
| Review requests and cash-pay campaigns | Limited | Yes |
We did not find a native HighLevel connector for WebPT, Raintree or Jane, and Jane says it does not offer an open API or API keys. Clinics usually let the front desk create the patient in the EMR at intake. Where an EMR can send events, HighLevel’s LeadConnector Zapier app or inbound webhooks can move basic contact data.
Protecting Plan-of-Care Completion
Most revenue in outpatient PT depends on patients finishing the visits in their plan of care. The CRM supports that with reminders, quick rebooking after a cancellation and a check-in when a patient goes quiet.
The messages never mention the injury or diagnosis. They say when the next visit is, how to reschedule and who to call.
Scheduling ownership matters here. If the EMR holds the real visit schedule, the CRM should not run a second, competing calendar for follow-up visits. We decide on the strategy call which system sends which reminder, so patients never get duplicate or conflicting messages.
Calls are the other leak. A busy front desk misses calls during check-in rushes, and missed-call text back keeps those patients from booking somewhere else.
HIPAA and Texting Setup for a PT Clinic
A standard HighLevel account is not HIPAA compliant. HighLevel’s HIPAA Compliance package costs $297 per month, includes a Business Associate Agreement, encryption of ePHI, audit logs and enforced MFA, and cannot be cancelled or switched off on a sub-account once enabled.
Your clinic remains the covered entity and still needs its own HIPAA program. The agency managing your account also signs a BAA with you, because it can see patient contact records.
Texting has its own rules. Forms need clear consent language, and your numbers need an approved campaign through our A2P 10DLC registration service before reminders and campaigns go out reliably.
KPIs and Discharge-to-Wellness Programs
Track four numbers: arrival rate, visits per episode of care, plan-of-care completion and referral volume by source. The first three come from your EMR; the CRM adds the lead side, such as speed to first contact and booked evaluations per source.
Reviews belong at the end of a successful episode of care. A physical therapy CRM can send the request after discharge, when the patient has seen results, and route any unhappy reply to the clinic manager first.
Discharge is also a natural moment to offer cash-pay services that do not need a referral or insurance, such as wellness visits, group classes or return-to-sport training. Only patients who opted in to marketing should receive those offers.

Physical Therapy CRM: Questions, Answered
A physical therapy CRM manages the patient relationship outside the treatment room: referral and direct-access leads, booking, visit reminders, texting, reviews and marketing. It works next to your EMR, which keeps evaluations, plans of care and billing. GoHighLevel combines these CRM tools with calendars, funnels and a shared inbox.
We did not find a native HighLevel connector for WebPT, Raintree or Jane. Most clinics keep the EMR as the clinical record and use GoHighLevel for leads, reminders and communication. Where an EMR can send data out, the LeadConnector Zapier app or HighLevel inbound webhooks can pass basic contact events.
Only with HighLevel’s HIPAA Compliance package, which costs $297 per month and adds a BAA, ePHI encryption, audit logs and MFA enforcement. Accounts are not compliant by default. Your clinic still needs its own HIPAA policies, and messages should avoid diagnosis or treatment details.
Every state allows some form of direct access to physical therapists, according to APTA, but many states set time or visit limits, and insurers may still want a referral. Your booking page should reflect your state’s rules and remind patients to check their coverage before the first visit.
It sends visit confirmations and reminders by text and email, offers a quick way to reschedule, and flags patients who cancel without rebooking so staff can reach out. Missed calls get an automatic text reply. Measure the effect through your arrival rate in the EMR before and after launch.
Yes. Each referral is tied to the referring physician, so reports show referrals received, evaluations booked and patients who started care by source. Clinics use that to plan physician outreach. See our GoHighLevel CRM hub for how we structure reporting across industries.
