Medical Practice CRM Built on GoHighLevel
A medical practice CRM handles the patient relationship your EHR was never designed for: answering inquiries fast, booking new-patient visits, sending reminders and asking for reviews. GoHighLevel USA delivers it as a done-for-you GoHighLevel setup for independent physician and specialty practices, with the clinical record kept where it belongs.

What Our Medical Practice CRM Setup Includes
We build the front-office layer of the practice in GoHighLevel and leave charting, coding and billing in the systems built for them.
Patient Inquiry Capture
Website forms, chat, missed calls and directory listings feed one inbox, so every new-patient request gets a fast and consistent reply.
Minimal Intake Forms
Pre-visit forms collect what scheduling needs, such as visit reason category and insurance carrier, while clinical history is captured in the EHR.
Online Booking
New and returning patients choose an open slot by provider or visit type, with confirmations and reminders sent automatically.
Recall and Follow-Up
Patients due for an annual visit or overdue follow-up are invited back, with message wording that never names a condition.
Review Requests
Satisfied patients receive a short request for a Google review, and staff get guidance on replying to reviews without confirming anyone is a patient.
Referral Source Reporting
See which referring physicians, directories, ads and search listings bring new patients, measured from inquiry to completed first visit.
Is GoHighLevel the Right CRM for Your Practice?
It fits independent practices that want a stronger front office without replacing a working EHR or practice management system.
- ✓ You run a primary care, specialty or multi-provider practice with its own marketing
- ✓ Phone calls and web inquiries go unanswered during busy clinic hours
- ✓ Your EHR patient portal is weak at reminders, recall and reviews
- ✓ You want to know which referral sources and ads bring real new patients
- ✓ You need a vendor who understands BAAs and keeps PHI out of marketing

Medical Practice CRM Guide: HIPAA, Messaging and Reviews
Where a CRM ends and the EHR begins, what HighLevel covers under its BAA, and the seven steps we follow for every practice.
Where a Medical Practice CRM Fits Beside Your EHR
Every practice already has a system of record. Whether it is athenahealth, eClinicalWorks, ModMed or another platform, the EHR and practice management system own charts, orders, coding, claims and statements.
A medical practice CRM owns a different job: the patient relationship before and between visits. New patients find you through Google search and Maps listings, insurance directories, booking marketplaces like Zocdoc, physician referrals and your own website, and each of those inquiries needs a quick, accurate response.
We draw the boundary in writing before anything is built. GoHighLevel keeps contact details, visit-type requests, communication preferences, consent records and conversation history, while diagnoses, results and treatment plans stay in the EHR.
Typical patient pipeline stages
- New inquiry: a form, call, chat or referral arrives and gets an immediate acknowledgement
- Visit requested: the patient picks a slot or asks for a call back
- Eligibility checked: staff confirm insurance in the practice management system
- First visit booked and completed: reminders run, then the patient is marked seen
- Recall due: the patient is invited back for annual or follow-up care
HighLevel’s HIPAA Package and BAA, Precisely
HighLevel does not treat accounts as HIPAA compliant by default. Its HIPAA compliance help article describes an agency-level add-on priced at US$297 per month that cannot be cancelled, refunded or disabled once purchased.
After the Business Associate Agreement is reviewed and signed, HIPAA mode is enabled on each sub-account that stores PHI, and that switch is also permanent. Agencies on any HighLevel plan can subscribe.
| Area | Handled by the HighLevel package | Still your practice’s job |
|---|---|---|
| Data at rest | AES-256 encryption of ePHI | Deciding which data belongs in the CRM at all |
| Access | MFA enforcement and audit logging | Assigning user roles and removing departed staff |
| Contracts | BAA between HighLevel and the agency | BAAs with your own agency and every other vendor |
| Covered records | Contact records and notes, custom fields, texts and MMS, voice recordings, email content and attachments, form and survey submissions, calendars and invoices | Message content, risk assessment, policies and training |
The agency managing your account must also be HIPAA compliant to sign a BAA with your practice. We confirm that chain of agreements before any patient data is imported.
PHI-Safe Messaging and the FCC Healthcare Exemption
Texting patients needs two separate checks. Carriers require a registered A2P 10DLC campaign, which our A2P 10DLC registration service files for you, and federal calling law adds its own conditions.
The FCC’s 2015 TCPA order created a limited exemption for free-to-the-patient healthcare messages such as appointment confirmations, pre-operative instructions, lab results and prescription notifications. Its conditions include texts of 160 characters or less, no more than one message per day and three per week, a STOP opt-out in every message, and no marketing or billing content.
Marketing texts, like a new-service announcement, fall outside that exemption and need the patient’s consent. We keep reminders and promotions in separate templates so the two are never mixed.
Answering Online Reviews Without Disclosing PHI
Review replies are a quiet HIPAA risk for practices. HHS’s Office for Civil Rights settled with a California dental practice, New Vision Dental, for $23,000 over patient information disclosed in responses to online reviews, and a Dallas dental practice paid $10,000 in 2019 over similar social media disclosures.
The safe reply never confirms the reviewer is a patient and never discusses care. We give your staff approved reply wording and route negative feedback to a private conversation inside GoHighLevel before it becomes public.
7 Steps to a HIPAA-Aware Medical Practice CRM
The page title promises seven steps, and this is the order we follow for every practice. The four build phases above are where the work happens; these are the decisions inside them.
- Map the boundary between the EHR, the practice management system and GoHighLevel
- Decide whether PHI will live in the CRM, and if so activate the HighLevel HIPAA package and sign the BAA chain
- Register texting through A2P 10DLC and write consent language for every form
- Write PHI-free templates for confirmations, reminders, recall and marketing
- Build the patient pipeline, calendars by provider and visit type, and minimal intake forms
- Set up review requests and a review-reply policy for staff
- Train the front desk, then report on inquiry response time, booked visits, show rate and referral sources
This is general information, not legal advice; confirm with counsel how HIPAA, the TCPA and your state’s rules apply to your practice.
Practice Growth KPIs Worth Reporting
Practices rarely lack data; they lack the few numbers that show whether the front office is working. We set up GoHighLevel reporting around five of them.
- Inquiry response time: minutes from a new request to the first human or automated reply
- Inquiry-to-booked rate: the share of new requests that become a scheduled first visit
- Show rate: booked first visits that actually happen, by provider and visit type
- Recall completion: patients due for follow-up who book within the recall window you set
- New patients by source: referring physicians, directories, search listings and ads compared side by side
Visit volumes and collections still come from the practice management system. The CRM side shows where demand starts and where it leaks.
Specialty Practices and Next Steps
Some specialties need their own pipeline design. A chiropractic office runs on care plans and personal-injury referrals, so it has a dedicated chiropractic CRM build, and more specialties are listed on the GoHighLevel CRM hub.
If your practice already has a GoHighLevel account that someone else built, start with a GoHighLevel tech audit to find PHI in the wrong places and workflows that send without consent. For build costs, see our GoHighLevel setup pricing.
Medical Practice CRM: Questions, Answered
A medical practice CRM manages the patient relationship outside the exam room: inquiries, booking, reminders, recall, reviews and referral-source tracking. It works beside the EHR rather than replacing it, so clinical documentation, orders and billing stay in the system of record while the CRM handles communication and growth.
Only with HighLevel’s HIPAA package, which costs US$297 per month at the agency level, cannot be cancelled once bought, and includes a BAA, ePHI encryption, MFA enforcement and audit logs. The practice is still responsible for its own policies, risk assessment, training and signed agreements with any other vendor that handles patient data.
No. The EHR remains the clinical and billing record. GoHighLevel holds contact details, communication history, consent and appointment requests. Keeping that boundary clear limits how much PHI sits in the CRM and avoids staff entering the same clinical details in two places.
The FCC’s 2015 healthcare exemption allows certain free-to-the-patient treatment messages, such as appointment reminders, under strict conditions on length, frequency, content and opt-out. Promotional texts are not covered and need consent. Your number also needs A2P 10DLC registration. Confirm your approach with counsel before launch.
Never confirm that the reviewer is a patient or mention any care details. HHS has settled cases with practices that disclosed patient information in review replies. A short, general reply that invites the person to call the office, with the real conversation moved private, protects both the patient and the practice.
Yes. GoHighLevel can take after-hours messages, send booking links and route urgent calls to an on-call number, and our GoHighLevel AI automation team can add AI chat or voice. We set those tools to collect contact details and visit requests only, never clinical advice.
